Accidental Death & Dismemberment Insurance

Your union membership entitles you to enroll for a one-year, no-cost accidental death insurance policy with the Hartford Life and Accident Insurance Company. Coverage provides benefits to your family if you die or to you if you are dismembered as a result of an accident.

Enroll now to take advantage of up to $20,000 of no-cost coverage because you are a union member:

  • $10,000 of coverage for death due to any type of covered accident at home, at work or while commuting or traveling anywhere in the world.
  • Plus $10,000 of additional coverage against accidental death from a covered accident at your workplace.
  • This no-cost policy must be renewed annually if you wish to continue receiving this benefit.
  • This product is currently unavailable to residents of Oregon.

For even greater protection, you may choose additional protection of up to $200,000 for an additional low group rate.

Eligibility:

  • No-cost coverage for one year: for union members only, ages 18 and over.
  • Enhanced coverage: member or family eligibility
    • Union members and spouse or domestic partner ages 18 and over.
    • Spouse or domestic partner and children can be combined into one family policy with the member.
    • Union member and spouse or domestic partner must both reside in the U.S.
    • Children are covered to age 19 (24 if full-time student).

Enrollment
To enroll either call 1-800-393-0864 or apply online at Union Plus. When the page loads, look in the right lower side of the page for the “Apply Now” button. Click it to start the enrollment process.

The new screen appears, fill in the required fields with the following:

Union: SEIU – Service Employees International Union

State: WA

Date of Birth: enter month, day and year from dropdown menu

When your selections have been made, click the “next” button, which will take you to the next screen where you will enter your member information.

Note: If you want updates on your benefits emailed to you, please make sure the box at the bottom is selected.

Once the information has been provided, choose “next” and you will be directed to the coverage page, where you have the option to choose additional coverage for a low group rate or click the “next” button at the bottom of the page to enroll in no-cost coverage.
